Hi Kim, Sorry I couldn't reply sooner. If you go to www.familytreemaker.com, they have where you can look up the person on the SS Death Index. They have a pre-written letter that you can print out and mail in to get this information. If the person is not listed, then you can just write a letter and mail it in and they will try and locate the application with the information that you give them. It costs $7.00 if you know the SS# and $16.50 if you don't know the SS#. The address to write to is: Social Security Administration Office of Central Records Operations FOIA Workgroup P.O.
