The hardbound book ($35) and the softbound index ($3) are available from McClain Printing Co but only limited quantities of the index remain. Shipping/handling is $3 per book US Postal Service: $6 for first selection, $3 for each additional title UPS. (All hardbound books are shipped UPS.) WV residents must add 6% sales tax on all sales.
