Zimmerman Jr. today announced new requirements necessary to apply for a certified birth or death certificate, which will go into effect Feb. 1, 2003. 'ÄúDue to growing concerns about identity theft, these stricter requirements will ensure security of personal information,'Äù Secretary Zimmerman said.¬Ý 'ÄúThese new requirements will strengthen the integrity of the Commonwealth'Äôs vital registration system for birth and death records.'Äù ¬Ý All vital-records requests now require that the department'Äôs application be completed and signed. Photo identification is required for public office, mail, facsimile and legislative requests. Telephone requests will no longer be accepted. Certified copies of birth records are generally necessary to establish age and citizenship for school, employment, travel, driver'Äôs license, Social Security and other governmental benefits.¬Ý Certified copies of death records are needed for insurance claims and other estate settlement matters. To apply for a certified birth or death certificate, applicants can visit the department'Äôs website at www.health.state.pa.us/vitalrecords to apply online, download application forms to apply by mail or via facsimile or obtain directions to the one of the Division of Vital Records'Äô public offices located in the following cities:¬Ý New Castle, 101 S.
