Zimmerman Jr. today announced new requirements necessary to apply for a certified birth or death certificate, which will go into effect Feb. 1, 2003. 'ÄúDue to growing concerns about identity theft, these stricter requirements will ensure security of personal information,'Äù Secretary Zimmerman said.¬Ý 'ÄúThese new requirements will strengthen the integrity of the Commonwealth'Äôs vital registration system for birth and death records.'Äù ¬Ý All vital-records requests now require that the department'Äôs application be completed and signed. Photo identification is required for public office, mail, facsimile and legislative requests. Telephone requests will no longer be accepted. Birth and death records in Pennsylvania are not open to public access, and the Division of Vital Records will continue to process applications for certified copies only from eligible requestors.¬Ý Eligible requestors are limited to the individual named on birth record (if 18 or older) or an immediate family member such as parent, grandparent, sibling, grandchild, spouse or child.¬Ý A decedent'Äôs family members or the legal representative of the decedent'Äôs estate may request certified copies of death records.¬Ý The new security requirements follow and apply to all requests received by the Division of Vital Records including those delivered from legislators and messenger services: ¬Ý¬Ý¬Ý¬Ý¬Ý¬ÝMail Requests -- Each request for a birth or death record must be submitted on a revised application for a Certified Copy of Birth Record or Certified Copy of Death Record.¬Ý This application form requires an eligible requestor to sign a 'ÄúStatement of Entitlement'Äù as proof of identity.¬Ý A photocopy of government-issued photo ID that identifies the person applying for the record is also required.¬Ý ¬Ý¬Ý¬Ý¬Ý¬Ý¬ÝInternet Requests -- Applicants possessing a major credit card may submit requests online by visiting the department'Äôs website at www.health.state.pa.us/vitalrecords.¬Ý Applicants are required to electronically sign the 'ÄúStatement of Entitlement.¬Ý A supplemental security/pin number is required in addition to the standard credit card number and the certificate will only be mailed to the requestor'Äôs credit card billing address.¬Ý ¬Ý¬Ý¬Ý¬Ý¬Ý¬ÝFacsimile Requests -- Applicants with a major credit card may also apply via facsimile.¬Ý Each request for a birth or death record must be submitted on the revised application form containing the 'ÄúStatement of Entitlement.'Äù¬Ý A photocopy of a government issued photo ID that identifies the person applying for the record also must accompany the request.¬Ý A supplemental security/pin number will be required in addition to the standard credit card number and the certificate will only be mailed to the requestor'Äôs credit card billing address.¬Ý ¬Ý¬Ý¬Ý¬Ý¬Ý¬ÝPublic Office Requests -- Each request for a birth or death record must be submitted on the revised application form containing the 'ÄúStatement of Entitlement.'Äù¬Ý Photo identification will be required from all applicants.¬Ý ¬Ý¬Ý¬Ý¬Ý¬Ý¬ÝLegislative/Messenger Service Request -- Each request for a birth or death record must be submitted on the revised application form containing the 'ÄúStatement of Entitlement.'Äù¬Ý Photo identification of the requestor must accompany each request.¬Ý All legislative pages/messengers will be registered with the Division of Vital Records.¬Ý¬Ý ¬Ý¬Ý¬Ý¬Ý¬Ý¬ÝTelephone Requests -- Telephone requests are no longer accepted because the health department cannot readily authenticate the identity of the caller.¬Ý The Division of Vital Records issues more than 548,000 certified copies of birth certificates annually.¬Ý Last year, more than 350 incidents of potential fraudulent use of birth certificates were reported to the Division of Vital Records by governmental agencies for further investigation.¬Ý Nearly 50 attempts to secure birth certificates by imposters were detected by staff in the past year and referred to law-enforcement agencies for action. Certified copies of birth records are generally necessary to establish age and citizenship for school, employment, travel, driver'Äôs license, Social Security and other governmental benefits.¬Ý Certified copies of death records are needed for insurance claims and other estate settlement matters. To apply for a certified birth or death certificate, applicants can visit the department'Äôs website at www.health.state.pa.us/vitalrecords to apply online, download application forms to apply by mail or via facsimile or obtain directions to the one of the Division of Vital Records'Äô public offices located in the following cities:¬Ý New Castle, 101 S. Ruby, I can use GEDCOM or if you just want to e-mail your "messy" copy to me, that would be fine. The information that I have, so far, I believe came from the Ancestral File. I have found that I cannot believe that 100%. I have to follow up and do my own checking to see if I agree with the other people's deductions. I have found many mistakes in there. I don't know the basis of their research. It may be the book of which you speak. I use them as a "skeleton" for my research, until I can "meat" it out with my own research. I know that the Tinglers are actually Dinglers from Germany. The hard pronunciation of the "D" sounds like a "T" to the non-German ear. That is how we got the Carr spelling on this side of the pond. In Germany, we are Karr. Jeff Carr of TX researching Ball, Barnes, Carr, Coley, Crow, Cunningham, Daly, Furse, Gibbs, Gregg, Hale, Hill, Killian, McCammon, McKnight, Merrill, Moore, Mundine, Myatt, Putt, Rowan, Shannon, Smith, Thompson, Tuckwood, Walker, Wallace, Wheeler, Yourie and allied lines
Jeff I can help with the Tinglers,What would be the easiest way to get the info to you,There is a lot on the web abt. the Virginia Tinglers and I also have it on a messy hard copy.I used the book Michael Tingler Desc. as a work copy.So nice to know someone is interested in the Tingler line.Plus I think George Miller is Thomas's father but can't remember at the moment as to where I found the info. Margaret Miller was probably born about the 1760-early 80s. I believe that she is mentioned as Margaret Kile in the Washington Co, MD, settlement of Adam Deets/Dietz (1809). The only Margaret Kile(Coil) I could find was the one married to Jacob. I may have chosen the wrong Jacob, but he was my most likely candidate at the time. Thanks for the correction. Will Delete his death and see what else I can find on Jacob Kile(Coil). Do you have an estimated birth date for Margaret Miller: 2-Margaret Miller (-) sp: Jacob Coil/Kile (-bef 29 Oct 1810) I am not sure if the Jacob Coil/Kile you have listed is her husband. The Jacob who died in 1810 was born about the same era as Margaret's father, Thomas Miller(1730's). Jacob Coil was born about 1735-1740. It appears Margaret MAY have married one of the other Jacobs. I don't think she died from old age, before 1809. Any ideas on her possible birth year would be a big help. Thanks! Peggy - Thanks for your reply. I have Eye's book on Rexrode. It exemplifies the problem I face in that it gives the birth year of George Rexrode as 1760 and then presents other information that seems contradictory. What I am trying to find is an official source for George's birth year. All this is complicated by the fact that baptismal records from Pennylvania give birth dates for children of Zach and Anna Maria both before and after 1760, but George is not included. Bill, I have the Rexrode book written by Walter Eye. He lists three children by the pioneer Johannes Zacharias Rexrode, (Feb. 22, 1725-1799) and his first wife Anna Marie Voegel, ( - died about 1760) Married Nov. 19, 1751 in Tulpehocken, Burke County, PA. Came to Virginia about 1760-1762. Then seven by his second wife Catherine _____. I am guess that Anna must of died in childbirth as he lists her death in 1760 and the birth of Johann George in 1760. Then there is another child listed as born in 1761, so he must of remarried very quickly. Johann George Rexrode (1760-about 1835) m. Margaret Elizabeth Hevener about 1780 - was in Pendleton County in Census of 1830, then to Highland County. Lived on headwaters of Black Thorn - records at St. Michaels Lutheran Church, Sugar Grove, WV., listed George as Haniorg - German for George ? I don't know if this information will help you at all, but might give you a place to seach for more records. If I can help you any more from this book let me know.
