T. Peters and H. B. Carden was published in 1926 and reprinted in 1972. The original copy did not have an index but one was available later. The reprint was published with the index included. I have a copy of both. I knew Blaine Carden personally and my Dad knew both of the men very well. There are numerous photos in the book. Hope this is not too confusing.
