I don't know about that form, but do you know which branch of the service? What is his name? Do you know if he was fighting in Europe or someplace like Iwa Jima? You can alway write a letter to the department of the Army or Navy, or Marine Corp. That's all I did for my Father, and I didn't have to fill out any forms. Post all his info on the list and I am sure some one will know where to send you, or might even have information on him. There are also many WWII websites including the WWII memorial, that if your Brother in laws name isn't there you can add it. I have added my Father and 2 Uncles.
