You need NATF form 85, available for free via email request at: http://www.nara.gov/research/ordering/ordrfrms.html#form%2085 They will send either a minimal number of pages or the whole packet depending on what you request. I originally requested the minimal packet on my WI ancestor Silas Payne, and then requested the rest of the packet. What I found in the rest of it was of great value. Silas spent decades trying to prove his disability claim, and the court records included transcripts of his testimony, in which he described how he was orphaned, where he was born, who were the neighbors of his parents, the surname of the family that brought him to Wisconsin, and how he spent his years up to, during and after his service.
