Sometimes the person supplying the information is a spouse, child, or even an in-law. I have a death certificate of my ancestor, and the information on it--other than date and place of death--was supplied by the daughter-in-law. I suppose they picked her because the wife was grieving, son was with her, and the daughter-in-law was the next best thing. They could have made a better choice. She got the place of birth wrong (it was his childhood home, not the actual place he was born, because the family moved when he was 5), she got the father wrong (name listed was the father's middle name that he went by), and the mother's maiden name was wrong. The mother married twice, and the name listed was not her birth name, but the name of the first husband that died in the war...which was what she married #2 under.
