I had a very helpful reply from A Nolan, with an application form enclosed. The form's heading reads SOUTH EASTERN HEALTH BOARD APPLICATION FOR BIRTH CERT.> SEND TO: The Registrar, Community Care Centre, Grogans Rd, Wexford. I hope this helps, I have scanned the form and would be willing send as an attachment if you would like a copy.
