Hi Randy, I'm not sure, although I think they are photocopied from an original book. I am so delighted to have the information, I don't mind a smudge here or there as long as I can read it easily---but the ones I have are very clear, not mis-aligned, etc. There was another place I forgot to mention, but it sounds like you live in MA and are probably already familiar with them: Higginson Book Co., PO Box 778 Salem, MA. 01970 or orders@higginsonbooks.com I haven't ordered from them yet. There are a couple of books I plan to order from them and have recently found the form I printed out then misplaced! Happy hunting.
