This is what I found from the Social Security Death Index: HESTER O'NEILL SS# 532-46-9672 b.15 Nov 1891 RESIDENCE: 98632 LONGVIEW, COWLITZ CO., WA d.August 1972 ISSUED: WA (BEFORE 1963) I got this from Ancestry.com You can go to your public library and use their computers and print out a form (I will tell you where and how) that you send with some $$ and you can send away for a copy of their actual application which has alot of info. Go to www.rootsweb.com and look for Social Security Death Index You put in her name and SS# and when you see it come up, look around and you will see "FORM or PRINT" and click on it and it will already have her info on the page. Print it out and send in the money for it and you will receive the info. Good luck.
