It must refer to the Eastern part of present day Virginia. The song was written April 20, 1927 by Buell Kazee. One of the earliest recordings was in 1934 by the Carter Family. I had always assumed that what was referred to was the Eastern part of the state of Virginia and that this song predated the existence of the state of West Virginia.
