I sent for my grandfather's (came to US in 1900), and it contained very little. Did not contain parents names... only that they were from Sweden. Actually it didn't contain his real surname (afternam) of Wenstrom either. It contained his chosen surname of Buren. For the few extra dollars, order the full application.
