It means four men with the last name Fletcher were living in that district. Were they all related? Who knows. Very likely, but not necessarily. It does tell me each of these men was heading their own household at least over age 16. How many tithes are listed in each household? (Those are white males over 16, not necessarily family members, and slaves.) You'll have to dig a lot deeper than a tithe list to sort out the relationships.
