I had a similar situation happen a few years ago. I mailed in the application with the required fee and months went by with no response. I went to the local SS Office here in Nashville and they were able to pull up what I needed. Don't think that I ever heard from my request to SS at all. Perhaps my visit to the local office cancelled my request.
