Hi Diane, Thanks for the message. I had trouble calling the number you listed, but I searched and found them on the internet; another phone number is: 1-270-275-4075 Central Time. Their e-mail is: email: sammcpub@aol.com The shipping charge is $4.50, and they don't take bank cards. I didn't ask about KY state tax since I don't live in KY. When I told him I would send him a check, he told me to wait until I got the bill, and that they would ship the book on Monday.
