I received this from the SSA, 1/11/01. I think we will still be getting the SS-5 applications that we are used to, if it can be located, and the Numident report, if they can't locate an application. It is not customary for a numident to be sent when an individual has requested a copy of the Application for a Social Security Card (Form SS-5). We apologize for any inconvenience this may have caused. If you would write back to us again using E-mail and provide the full name and date of birth of the person whose application is being requested, we may be able to locate that Social Security number (SSN) and provide a copy of the SS-5. Please provide a telephone number where we may contact the person, who made the request, during business hours, i.e., 6 a.m. to 2 p.m. (eastern time), in the event we are unable to locate the SSN in question with the information that was provided. PLEASE NOTE: There will be a charge for the cost of searching our records even if we are unable to locate any information on the person you are asking about. The fee for searching our records is $7 when the SSN is known and $16.50 when the number is unknown or is incorrect.
