    3. I saw this on Gen Forum, and I thought you might want to see this, if you haven't already, about SSI Aplications now being sent. This was posted on the General GenForum Board: (From Gay Mathis) Nan Cattell Previously I have used the SSDI to order an SS-5 (Application for Soc. Sec. Number) & I have received a copy of the original application giving me my family members address at the time of the application & the names of their parents. I recently ordered another SS-5 for another family member & I received what they called a Numident report which gave me absolutly NO helpful information i.e. no address, no parents names, only the name, SSN & DOB which I already knew. Has anyone else had a problem getting the original copy of the SS-5? Any suggestions on how to get the original application? Elviria Hi,did you go to ancestry.com-print out the form letter for the person,and send it in?I've sent for 7 or 8, at various times-always have received the SS-5 form-filled out at time individual filed for S.S. #.Fee-$7.00-personal check-o.k.;no sase,needed. Elvira Nan Cattell Yes, I did go to Ancestry.Com & I sent my check, & in the past I have always gotten a copy of the actual SS-5 form. The letter I got was from a Darrell Blevens, Freedom of Information Officer, Social Security. It states that they are only sending out "Numident" reports now which are their computer printouts. This report gives NO valuable information & is certainly not worth the $7.00. Elvira Well,I'm sorry to hear this.You're right;it's not worth $7.00.It's really too bad;it was a good source of info.
