I had a similar experience. They said I wasn't entitled to cause of death, so that's why they wouldn't send me the copy. I re-sent the form and left the "show cause of death" box unchecked; they kept my money, but promptly returned a letter saying they had no record of that death certificate. I waited a couple of years until that year was added to the Archives files. It happened that I was in Nashville just after that year was sent to them and I could find no record of my GG-grandmother's death. Maybe we're off on the year, or maybe that's just their way of telling us not keep trying back because they lost the records! ;) If you want to contact me off the list, I do have the name of someone who goes to Nashville and will look up records for you for a reasonable fee. Please complete the attached form and return with the fee of $12.00( Cause of death cannot be verified) Those are the exact words. I sent saying I was needed a copy, gave name, dates and they had also said I had to have a copy of a government issued ID, so I copied my drivers license , sent my money and got this letter back...any suggestions for me would be appreciated.
