Help please. does anyone know if the cost for getting a copy of a social security application if you do not know the number is still $16? any idea of what the turn around time is? also can't find where I was to get this info on the Net. does anyone have the url? thanks for any help you can give me!!     3. I found the following as a reply to an inquiry after a little searching. Les Tate ---------------- To obtain a copy of the original social security application of an ancestor, write: Freedom of Information Office, 4-H-8 Annex Bldg., 6401 Security Blvd., Baltimore MD 21235 Request a copy of the SS-5 form. It includes great info like birthplace, mother's maiden name, etc. The person must be deceased and you should include the date of death to prove this. The cost is $7 with a social security number (from the soc. sec. death index) and $16 without. If you don't have the number, give as much info as you can to help narrow the search through similar names. Allow months for an answer.
