I was directed to http:www.gov/foia/foia_guide.htm If you go to that site, it will print the letter for you and all you have to do is send it along with a check for $7 to Baltimore. Can't be any easier than that. I had mine back in about 4 weeks. Good luck to you in your search. It did help that I already had the SS # for both my father and grandfather and I asked for both applications so I really hit paydirt.
