You can find the birth date and death date on the Social Security index on Ancestry.com site. It also has a form letter to the social security asking for information on the orginal form. The site is http://ancestry.com/search/rectype/vital/ssdi/main.htm The cosst of the inforation from Social Security if $10.00.
