It states Certified copy of an entry of birth. For the purposes of National Health Insurance Acts, 1911 to 1920 or of the Unemployment Insurance Act 1920. On the reverse of this are the details of another person. I was wondering if it rings a bell with anyone. It is for a George Albert Henry what looks like SIVYER b 24 August 1912, place of birth 12 Warwick Street, Southsea. Fathers name Harry Ernest SIVYER occ Labourer.
