Call the VA Admin in Washington at 1-800-697-6947 or write, but the best way to get VA FORM 40-1330 "Application For A Standard Headstone Or Marker" is to call first and then go visit your local veterans service officer to pick up one. The regs say that the tombstone has to not be a replacement stone and only for an unmarked grave but if you have the cooperation of the cemetery officials perhaps something could be worked out!!!!! If you want to buy an Iron Cross of the highest caliber, from a Tenn. SCV Unit, for $75, then let me know and I'll get you the phone number. They are shiny cast iron but with no finish. I have a friend that has a auto paint and body shop and he gave them multi-coats free heavy duty primer and black enamel whenever he finished a job and before he cleaned his spray guns and I then hand painted mine with a customized paint/color scheme job. Hope that this helps----Julian --------------EF27BB34CCDE1EB7D278CEB7--
