This asks for the parents names, date of birth, residence and occupation They will send an abbreviated printout for $16 but I don't know what information is included would assume most of the pertinent information, and wouldn't have his signature. There us a FTM file with an Emery on Disk 9 but I don't have that. Maybe someone else has it.
