Those are caused by multiple links, an error that is correctable from within RM, as you have discovered. I'm not sure they should be called "facts" at all, or perhaps, in this context, what we are really discussing is "events", and that is the term I should have used. Identical events across multiple persons may not even be possible within the database design; I've only given this question a cursory thought but realized that it was not clear enough for my understanding of the problem.
