Your full name 2. your mailing address where you want the kit delivered 3. your phone number 4. the email address you want Family tree DNA to contact you with Please don't reply to this email with your personal information. Send it to my personal email address (you should already have that). Also you can go ahead and prepare your Reeves family line and send that along so it can be posted to the project website.
