Fourth Edition was printed 1997. 481 pages This is probably in many libraries and would help you locate where the meetings were held if you have the name. His address in 1997 was 1800 Star Bank Center, 425 Walnut Street, Cincinnati, OH 45202-3957 for anyone who may be interested in trying to locate a copy for their own use. It's possible there is a later edition by now or maybe he found all the meeting locations. What a tremendous work! I hope he may have copies for your purchase.
