    3. If this is a Polish record, I picture the groom's name followed by his father's name and then mother's name possibly indicating _her_ maiden name or that SHE is the daughter of so and so. This column would be followed by other info and then the bride's name followed by her father's name and mother's name possibly indicating the _mother's_ maiden name or that she is the daughter of so and so (listing _her_ father's name). The nice thing about these formats is that you go back at least another generation or two. Now, if your situation is different than the above, I think we'd need more info. The best thing to do is to post a scan of the document so we can take a look at it. Let the list know if you need a place to post the scan.
