My question this time is what form do you use to submit to SSA to request the original application of your ancestor. The form I need a copy of is the SS-5. An article on the rootsweb site written by George Morgan in 1998 says request from SSA form L-997. When you go to the SSA website and type in form L-997 says not found. Anyone know the number of the correct form? Lindsey
