My sincere thanks for your response to my *scream* for help. Have checked the site that you sent & have d'loaded a form to submit to St. Dept. of Health in New Castle. You asked what county my person was b in - he was b 1896 in Wellsboro, Tioga Co. I now have the s-mail add. for that place also (taken from the site)& will write to them.
