Herman of Clearfield, Pa., died on Thursday, September 23, 2004 at Clearfield Hospital. She was 43. She was born on March 4, 1961 in Port Jervis, a daughter of Hazel (Hegarty) Coon of Clearfield Pa. and the late Glenn D. Coon. Ms. Herman was a private duty nurse's aide having worked with many families within the area during the past 7 years. In addition to her mother, she is survived by a son, Travis Herman at home; two brothers, Kim T. Coon and wife, Vickie of Mechanicsburg, and Kevin G. Coon of Clearfield, Pa.; a sister, Cindy J. Kozykowski of Clearfield, Pa., a fiance, Melvin Gosnell of Clearfield, Pa.; and several nieces and nephews. In addition to her father, she was predeceased by her husband, Larry Herman in 1989. Funeral services will be held at the Kevin A. Beardsley Funeral Home, Clearfield on Monday at 10 a.m. with Dr. Duane A. White officiating. Interment will follow in the Beulah Cemetery, Ramey.     2. My question this time is what form do you use to submit to SSA to request the original application of your ancestor. The form I need a copy of is the SS-5. An article on the rootsweb site written by George Morgan in 1998 says request from SSA form L-997. When you go to the SSA website and type in form L-997 says not found. Anyone know the number of the correct form? Lindsey

My maternal grandmother was employed after her husband died in 1942, and she collected SSA benefits from her retirement until she died in 1960, but she is not on the list. Yet my great great uncle, who died in 1940, IS on the list. Go figure. Frankly, I think the reason people are omitted on-line has more to do with SSA having to manually input early data, than anything else.
