The link for the form that should be used to request certificates can also be found on this webpage. Go down almost to the bottom where the link is to the PDF form you can print and mail in. Look for the words "download the death application form". These records are grouped in alphabetical order in some years and in soundex order in other years. To determine the soundex code for the surname that you are searching you can go to the link at: http://searches.rootsweb.ancestry.com/cgi-bin/Genea/soundex.sh Some names are alpha by first name. Those names are part of the soundex years of the death indices. You can determine the soundex code of the surname then look in those years under the soundex code by first name. Searchers have experienced the indices, which are in a PDF format, slow to load. Be patient if at first it appears that the indices are not loading.
