What you may have to do [I know because I had to do this for getting a passport] is have 2-3 reliable witness who have known you by your current name swear in front of a lawyer or a notary that you have never used your birth certificate name but that you are that same person! Ask them if they will accept that [the US gov't does for official passports]. Cora
