They take care of submitting the form to the proper department/address. Your certificate will go back to them in fairly short order and they will mail it to you. The other week I requested, with proper monies,a Fetal Death Certificate and received it in about 10 days. Since I'm in PA, this will only work for this state. Other states I must mail myself with a generic form which usually is standard in all states - hopefully they have the correct address in their files! If you get no response from another listmember, call your local rep on Monday & check the address with them. -Linnea P.S. In PA, if you are a active military or a veteran you can request one birth/death certificate for you or immediate family members at NO CHARGE!! This may be true in other states as well. All you need to add is service number & rank, which is filled in on the reverse side of the request form.

Thanks for any help.Bev I found the grave of my Cattarina (Zeller) Miller( 4th great-grandma)!She is buried at the same cemetery as her husband,Michael Miller at Salem Ref.church in Bethel, Berks county, close to Rehrersburg.Her stone is a few rows away from his . She died young! It says, Cattarina Zeller, wife of Michael Miller, b- June 1811 d- July 1849 38 y/10m/27 days That is what the dates appeared to be,(stone is worn) but I also sent a letter off to the church for their help to get the dates from their records.She was married to Michael Miller b- 7-12-1804/died 7-15-1874.Who are her parents, need Michael Miller's parents also,Thanks!
