Allison has provided high quality scans of the sheet she was sent. Unfortunately the copies were not high quality. Apparently these are copies of copies of copies, plus text on the reverse shows through and adds to the confusion. If anyone would like to take on the challenge of creating a table (in Word, Excel or Access) that I can then convert to a web page, I would very much appreciate it. The material will get posted faster that way. One table for each of the two main sheets (Avenues A - E, and Avenue N) would be needed - thus two volunteers would really speed things up.
