I don't have a blank card but I do have several cards for family members. Mine read as follows. Name of Soldier Name of Applicant Widow Minor Service Date of Filing Class Application No. Certificate No. State From Which Filed Attested Remarks Under Class it does say Invalid Widow Minor Hope this helps. This column also has sub headings which appear to say Invalid, Widow and ??? I just can't read the last one ......maybe Minor or Married??? I would like to know what the unknown name is as it appears to have the date of 23 June 1868 in it but, I don't know what the date is for. Does any one have a good quality blank Pension card they could email me. Or at least tell me what the column names are? Any help would be greatly appreciated.
