Mary Lucille PTAK, 65, of Uniontown, Pa., died Tuesday, January 13, 2004, in the Beverly Health Care Center, Uniontown, Pa. She was born May 15, 1938, in Masontown, Pa., daughter of the late Frank B. PTAK and Agnes MUTNANSKY Ptak. Her parents preceded her in death. Mary completed nursing school at the Uniontown School of Nursing and also Duquesne University and became a Registered Nurse, and she retired as a Registered Nurse. She was a lifelong member of the St. Joseph Roman Catholic Church in Uniontown. She was also a member of the Uniontown Senior Citizen Center and a member of the 500 Club. Mary is survived by a brother, Frank S. PTAK and wife Arleen, of Brownsville, Pa. and a sister, Dolores E. Premus and husband Robert, of Beavercreek, Ohio; a nephew, Robert F. Premus; nieces, Francine S., Alyssa S. PTAK and Deborah S. Premus; great nephews and nieces, Jacob Newlon, Whitney PTAK and Dustin Franks. Visitation will be held in the STEPHEN R. HAKY FUNERAL HOME INC., at Sylvan Heights Cemetery, a Dignity Memorial Provider, 603 N. Gallatin Ave., Uniontown, Pa. 15401, from 7 to 9, Friday, January 16, 2004, and until 9 a.m., Saturday, January 17, 2004, when a Blessing will be held in the Funeral Home with Fr. Alexander L. Pleban. A Mass of Christian Burial will be celebrated at 9:30 a.m., in the St. Joseph Roman Catholic Church, 180 Old Walnut Hill Road, Uniontown, Pa. 15401, with Fr. Alexander L.
