Helen M. Horak, 82, of Uniontown, Pa., died Tuesday, Feb. 3, 2004, at the Uniontown Hospital. She was born Feb. 25, 1921, in Youngstown, Pa., the daughter of the late George HEINTZELMAN and Anna NICODEM Heintzelman. In addition to her parents, she was preceded in death by her husband, Frank P. HORAK, on Dec. 17, 1988; six brothers, Glenn, Elmer, Lawrence, Earl, Charles and Carroll HEINTZELMAN; and three sisters, Florence McShane, Dorothy Collier and Kathryn HEINTZELMAN. She was a member of the St. Paul's Evangelical Lutheran Church and the Wismer Sunday School Class. Helen is survived by a brother, Robert HEINTZELMAN, of Latrobe, Pa.; a sister-in-law, Rae Heintzelman of the state of California; a brother-in-law, Lee COLLIER, of Uniontown; several nieces and nephews; and a special caregiver and niece, Janice Erminio and husband, Nick, of Hopwood, Pa.
