Usually when this happens, your individuals list has been inadvertently dragged off the screen to the left. Open the Individual List screen and place your mouse pointer over the left edge. If it turns into a double arrow, click the mouse button and drag it to the right. You should see it come back as you drag further to the right. Robert, Richard's solution usually works. If not, try right-clicking in the heading space and adding the missing fields back in.
