Personally speaking, when my mother was killed in an auto accident, I was hard put to remember anything at all, including her name, my name, her date of birth, etc. My 11-yr old son had to prompt me. Fortunately the funeral director was a very patient man and I came back the next day better prepared. Otherwise, I think there would be many blank spaces or "unknown" entries! Sharon Bakersfield, CA
