The Court House at Media have copies of the naturalization papers and they also can get for you a Petition document which just shows the same information s the Naturalization paper. The do not have a form that the personwould have filled out with information before the petition was granted. At least they are not aware of such a paper if indeed there was one. I would think that you would have had to fill out some form before they would even start the process.
