Read very recently that the death certificate is generally made out by the undertaker and is only signed by the doctor, so don't always blame the doctor. My mother was born in Mass. and was to be named Alice Mae. Her father went to the town office to register the birth. 65 years later when she had to get birth certificate she found she was named Alice Mary -after his mother ! Ralph Weller
