I believe that geologically, South Walls is an island. The connection to North Walls on the island of Hoy is essentially a sand-bar, which used to be covered at high tide, but has now been improved by man and had a road constructed on it. In the same way, the Brough of Birsay is an island to which you can walk at low tide.
