Hi Sue, I think it looks like Wm. Culears Mentialley disordered The surname 2nd letter looks like u or even a but the connector to the next letter is at the bottom so I don't think it's an o. The 3rd word could be Martialley but weren't men's "infirmities" supposed to be listed . I'm fairly sure about the last word.
