While it is possible that you could get a copy of the DC with just the name and year, it sure would be chancey, especially when they keep your ten dollars no matter if they find it or not. I have gotten many DCs with just the name, full date and county of death. I usually leave the funeral home space blank because I don't know, so that is no problem there.
