They are open from 8:30 am to 4:00 pm, Monday through Friday and may very possibly be able to provide the birth or death record while you wait. Q: How can I obtain a certified copy of my birth certificate? A: There are several options at your disposal for obtaining a certified birth certificate. Blank applications are available at the State Vital Records offices in Tulsa and Oklahoma City. Beginning July 1, 2004, certified copies can be obtained from the Oklahoma State Department of Health's Tulsa Division of Vital Records at 315 South Utica, Phone: (918) 594-4840. There was a note on the door at the Tulsa Vitals office saying they had closed that facility and that all requests had to go through OKC. Now, the office we went to was: Vital Records Service, Oklahoma State Department of Health, Tulsa Office 108 N. Greenwood Street, Tulsa, OK 74120 918-582-4973 Hours: 8:30 am-4:00 pm M-F Evidently they opened the new office after we made our trip to Tulsa. So, the address on Greenwood is invalid now. I called the number of the office on Utica, and it is inside the Tulsa DHS office. Some counties still show the Greenwood address, so everyone should now, they can still get them in Tulsa, but have to go to Utica DHS office.
