    3. Q: How can I obtain a certified copy of my birth certificate? A: There are several options at your disposal for obtaining a certified birth certificate. Blank applications are available at the State Vital Records offices in Tulsa and Oklahoma City. Beginning July 1, 2004, certified copies can be obtained from the Oklahoma State Department of Health's Tulsa Division of Vital Records at 315 South Utica, Phone: (918) 594-4840. Certified copies can also be obtained by forwarding your request, a $10.00 check or money order made payable to OSDH, and a self-addressed stamped return envelop to the Division of Vital Records Oklahoma State Department of Health 1000 N.E. 10th Street Oklahoma City, Oklahoma 73152-3551 The date they give here is about the time we went to Tulsa. There was a note on the door at the Tulsa Vitals office saying they had closed that facility and that all requests had to go through OKC. Now, the office we went to was: Vital Records Service, Oklahoma State Department of Health, Tulsa Office 108 N. Greenwood Street, Tulsa, OK 74120 918-582-4973 Hours: 8:30 am-4:00 pm M-F Evidently they opened the new office after we made our trip to Tulsa. So, the address on Greenwood is invalid now. I called the number of the office on Utica, and it is inside the Tulsa DHS office. Some counties still show the Greenwood address, so everyone should now, they can still get them in Tulsa, but have to go to Utica DHS office.     3. Went to Tulsa a few months back to help a friend obtain a birth certificate. That office is now closed. ALL requests must go through OKC and you HAVE to send a copy of your drivers license with all requests, or they will be returned. Thought everyone might want to know as a lot of counties list the Tulsa office as a place to get these, but unfortunately this office doesn't exist anymore. I was looking for information on the Comanche County cemeteries and Sharon thought she gave you a book that listed them. I found a list at the Lawton library and found the Pecan cemetery. Plan to find all the public cemeteries in Comanche County this next year. A cousin saved obits from the Lawton newspaper from 2002-2004 and I finally got them online.
