It could be some type of burial insurance from a local funeral home. In this area of southeastern OK the local funeral homes would sell small burial policies that provided for a certain amount to be paid toward burial costs or funeral costs. These were usually only good through the funeral home that sold them. The ones that I am familiar with during the 1960s and 1970s would indicate that you were a member of such and such burial association, which was usually the name of the funeral home where you purchased the burial policy.
