Are you sure you didn't misread the form? I believe the Stark Co. District Library's request forms ask for the requester's name and address in order to send the copies to you. You will get an "invoice" stating how much is owed for the copies sent. By the way, this library (especially its genealogy department!) is top-notch!! C.
